If you do not see your vision plan listed above, please contact our office or refer to the provider list. Vision plans that we accept may not be listed due to the large number of name variations and affiliations. Madeira Optical accepts plans from VSP, EyeMed, Aetna, Anthem Blue Cross Blue Shield, Avesis, Cigna, Humana Choice Care, Superior Vision, Medical Mutual of Ohio, Ohio Health Choice, Outlook Vision, Vision Benefits of America, Vision Care Plan, Vision Plus, United Health Care and others. Our largest web referrals are from Cincinnati Bell, Children's Hospital of Cincinnati, Christ Hospital, Convergys, Fifth Third Bank, Johnson & Johnson, Procter & Gamble and The University of Cincinnati. Coverages will vary based on your specific vision plan. Please contact your plan for information or we will gladly verify coverage for you before your eye examination. If you would like, we will also help you with self-submit plans and out-of-network paperwork.
